Delivering care where people live, work and learn rather than in hospitals or other acute settings, nurses and midwives in our communities provide holistic, relationship-based care across all age groups. Community and primary care nursing staff have a unique role in tackling health inequalities, meeting the health needs of their local population and in the prevention of ill health, through health promotion and early intervention. They support people with long term conditions and people in recovery, preventing hospital admissions and supporting timely discharge. 

Working in a variety of locations from a person’s own home, GP surgery or clinic to community centres, schools and prisons, they understand their locality and adapt, target and tailor their nursing care to meet local needs.

This award aims to recognise those who demonstrate a commitment to delivering high quality, person-centred healthcare in Scotland’s communities. Our nominees will have developed and implemented innovative approaches to delivering care closer to home. Nominations illustrating sustainability, continuity and consistency for people receiving primary or community-based care, are particularly welcome.

Who can be nominated?

The Award is open to individual registered nurses, midwives or teams working in any primary care or community setting including the independent health and social care sector, third sector, NHS or local authority. They must demonstrate responsiveness to local needs, collaboration with patients, families and other professionals and a commitment to improving health outcomes.

What is the criteria for consideration?

Each nominee must clearly demonstrate:

• a commitment to person-centred care, innovation and delivering high-quality services that make a difference to the people receiving care
• the positive impact of their work for individuals and families receiving care, colleagues and their local community
• the use of a credible evidence base and or developing an evidence base to underpin the work for which they are being nominated
• how they have addressed the unique challenges of providing care within local communities and how this contributes to local and national strategy.

The nomination process opened on 14 September and will be open until midnight on 20 November 2026.

Once the nominations process closes on 20 November, the nominations are sent to our expert judging panels in each category where they go through several rounds of whittling down to a final three or four candidates.

The final stage of judging is then an informal meeting between the judging panel and the finalists online via Microsoft Teams. The interviews last around 30 to 40 minutes and finalists are sent information in advance to help them prepare. The interviews are due to take place on the following dates and finalists will be contacted to arrange a suitable time:
